Care Transitions: Outcomes and Readmissions

Readmissions negatively affect patient outcomes, waste taxpayer dollars, and increase healthcare system financial burdens. While this is not a new problem, the Affordable Care Act has provisions to decrease hospital readmission rates. One focus area is the transition of care between facilities or providers, or “care transitions.” This course presents concepts of “transitions of care” (or “care transitions”) to improve patient outcomes and decrease avoidable readmissions.
